Patient Forms
To speed the check-in process, we encourage all patients to complete their registration forms in advance and bring them to their appointment. These forms below are editable PDF files. Please download the files specific to your exam, complete the information, provide a signature, and give the printed copies to the front desk at the time of your appointment. All patients need to fill out the Patient Registration form in addition to their exam form. New patients should also sign and bring the Notice of Privacy Practices, and PHI release form with them. To speak directly with a member of the AAR scheduling team, call 703.824.3260 – Monday through Friday from 7:30 a.m. to 5:00 p.m. Please have the following information handy:
- Your doctor’s order
- Current insurance information
What to Bring
Please bring your doctor’s order, your current insurance card, a valid photo identification card and a listing of any medications you are currently taking.
Missed Appointments
At AAR, we realize that life happens, and you may have to reschedule or cancel your appointment. Please call us at least 24 hours in advance. To cancel or reschedule an appointment, please call 703-824-3260.
Late Arrivals
If you are more than 15 minutes late for a scheduled appointment, we will try our very best to work you into the schedule, however it may be necessary to reschedule your exam so as not to inconvenience our other patients. Upon check-in, one of our Patient Service Representatives will inform you of the anticipated wait time. At that time, you may choose to wait while we work you back into our schedule or you can reschedule your exam.

Health Plans and Insurance Information
Alexandria Radiology participates with most major insurance plans to provide convenient access for patients at our imaging centers in Alexandria and Woodbridge. Health plans can periodically change; we recommend patients always confirm participation with their plan administrator prior to scheduling.

There are a few exceptions. We do not accept United Healthcare Community Plan, Aetna Better Health, Optima Health Medicaid, Maryland Medicaid, Humana Medicare, and Medicare Advantage, to name a few. If you have any questions, please contact your benefits administrator, and always check that AAR is in-network for your plan.
Request Your Medical Record
Please notify our office 48 hours in advance to arrange for a CD pick-up. If you have not called ahead to pick up your CD, we will be happy to accommodate your request however you may experience a delay as we process your request.
To arrange for release of your images on CD, please complete the Authorization to Disclose Health Information form and submit it to AAR.
Please call the center where your exam was performed to arrange for CD pick up or mail delivery.

As a service to our patients, AAR will provide one set of images to referring physicians at no charge. Additional copies are available to patients for a fee of $7.00 per CD.
In compliance with HIPAA regulations, CDs will not be released to anyone other than the patient without authorization. If you are picking up images for someone other than yourself, you will need identification and written authorization from the patient. If an authorization note is already on file at AAR, only identification will be required.
