Case 1

History:

22 year old asymptomatic female with abnormal EKG

Findings:

Chest radiograph showed abnormal contour and bulging of the left heart border. CT and MRI showing enlargement of the left atrial appendage.

Diagnosis:

Congenital diverticulum of the left atrial appendage.

Discussion:

Atrial diverticulum can be right or left sided. Since no degenerative changes are seen histologically, they are though to be congenital. They can be silent and be recognized on routine chest X-ray, or can produce arrhythmias, mostly supraventricular or embolisms. Surgery is usually recommended to prevent thrombus formation.
